Peer Navigation Project
Another Life Foundation is dedicated to helping those battling with mental illness and suicidal behaviors.
Our Peer Navigation Project provides navigators who are dedicated to mentoring and assisting those in the lower income bracket, and who are historically known to have less power or privilege than others, achieve health and wellness.
Most navigators are peers whose experiences have been similar to those of the community members they are assisting.
They will provide mentoring and access to resources/services to better their lives, thus providing hope and resilience to those in recovery from mental health disorders.
These navigators have expert knowledge of all the relevant facility- and community-based services available for each community member.
Their duties include:
-
a listening hear/mentoring,
-
referrals to health care providers, and other care and support services,
-
accompaniment to appointments (including support with transportation),
-
adherence support,
-
routine appointment reminders,
-
follow-up for missed appointments,
-
tracking of those lost to follow-up, and
-
assistance for those needing help applying for Social Security benefits.
-
Our vision is a world of improved health and wellness.
-
Our mission is to insure all Coloradans have equal access to health and wellness resources/services.
Our navigators are also available to staff an office space at any community organization, to better assist clients in need.
